Starting April 1, the Portland records office will be open Monday through Friday from 8am to 5pm Pacific for walk-in records requests. Please bring a photo ID and a signed authorization form. Requests for records older than 2015 may take up to five business days while we retrieve them from off-site storage.
Online requests through the patient portal are still the fastest way to get your records.
